An increasingly ageing population, the increase in diseases such as diabetes, COPD or heart failure, and a growing pressure on primary care those people who live with a chronic illness. This is not a question for the future: it is an urgent matter for the present.  <\/p>\n<p>In this context, a recent study published in the Sanitary Gazette (Scielosp, 2025) analyses how digital technologies are beginning to be integrated into healthcare. a15&gt; are beginning to be integrated into health policies in Spain to improve precisely this type of to improve precisely this type of care. The question at the heart of the matter is simple, but ambitious: <strong>can digital tools help us to offer care? a40&gt; help us to offer more efficient, more continuous and more humane care to chronic patients?<\/strong> <\/p>\n<p>The authors of the study point out that traditional monitoring models\u2014regular visits, face-to-face consultations, poorly guided self-care\u2014are no longer sufficient to manage the complexity of chronic conditions. People living with long-term illnesses need closer, more personalised care, but also a system capable of anticipating and preventing decompensation and better coordinating resources. This is where digital technologies come into play.  <\/p>\n<p><strong>From theory to practice: what is really happening<\/strong><\/p>\n<p>The analysis reviews various initiatives and programmes already in place in Spain: from monitoring apps for chronic patients to telemedicine platforms, early warning systems and tools that allow professionals to access clinical data in real time.<\/p>\n<p>Each of them points in the same direction: facilitating continuous monitoring that does not depend solely on face-to-face visits. For many people with chronic illnesses, this means something as simple\u2014and as important\u2014as avoiding unnecessary travel, receiving recommendations tailored to their situation, or detecting in time a worsening of their condition that could lead to hospitalisation. <\/p>\n<p>The study highlights a key point: good technological integration not only benefits the patient, but also the healthcare system. a9&gt; the patient, but also the system. More proactive and monitored care reduces emergencies, avoids admissions that could be avoided and frees up time in primary care, one a28&gt; of the links most strained of the healthcare package. <\/p>\n<p>This is not achieved with &#8220;more technology&#8221;, but with <strong>technology well integrated<\/strong>, aligned with professionals and tailored to the real needs of the population a19&gt;.<\/p>\n<p><strong>The challenges ahead<\/strong><\/p>\n<p>Not everything has been resolved.
